  2. JAMB result/acknowledgment slip
  3. A’ level results, diploma, or IJMB (as applicable to you)

  7. Certificate of birth/declaration of age

Ensure that you pack these documents in a quarto-size envelope, and don’t forget to label it with your name, department, and JAMB registration number.
